911 Narrative

ALPHA 44 was dispatched to 123 Main Street for the unknown medical involving an 84 year old male. Upon arrival, EMS consulted fire who had informed EMS that the patient had suffered a mechanical fall in his bathroom. EMS then made their way to room 291 where the patient's CNA informed EMS that the patient had “slipped on a towel while trying to use the bathroom” earlier that morning. He had been on the floor for about 10 minutes and was complaining of left-sided hip pain.

Patient was initially found lying supine on the bathroom floor. Patient presented as A&Ox2. Initial assessment revealed patient had a patent airway, normal respirations and adequate circulation. EMT J.Smith then held immobilization of the patient's C-Spine before EMT W.Ralf measured and applied C-Collar. After assessing the patient's entire body for DCAP BTLS, it was noted that there were severe deformities in the patient's pelvic area. Both EMT’s then applied a tight pelvic wrap using a towel provided by the CNA. The patient was transported onto the stretcher via scoop stretcher and was then secured using straps x5 and rails x2. Patient was loaded into ambulance without incident.

En route to Lahey ER, patient’s vitals were initially recorded as stable. Patient's pupils were equal and reactive to light. HEENT unremarkable. Patent was PWD. At this time, the patient denied any CP, SOB, NVD. Patient rated his pain at a 7 on a scale from 1-10. Patient stated that he did not lose any consciousness and did not experience any headstrike. However, as the patient was over 65, C-Collar was measured and applied regardless. Patient described hip pain as a throbbing, sharp sensation. Patient condition remained the same throughout transport and was reassessed every 5 minutes until transport was complete. A brief report was called into Lahey when the ETA was around 10 minutes.

Upon arrival at Lahey, the patient was assigned to A Pod and placed in his appropriate bed via use of the draw sheet maneuver and assistance from EMT’s x2 as well as ER staff x2. A report was given to his assigned RN. Because the patient was not A&Ox4, a receiving facility signature was obtained at the same time. Facesheet was later obtained once care was officially transferred. END. SM.
